MEMBERSHIP APPLICATION
Thank you for starting the membership process. We will be in contact with you shortly. Until then make sure to prepare the following documentation:
1. Appointment letter from Fire Chief on Department Letterhead acknowledging your role as chaplain, if currently appointed.
2. A letter of endorsement from Pastor or Church Leadership who knows your testimony.
3. Copies of any applicable ordination/training certificates.
4. Copy of your driver’s license.
5. Current photo of yourself.
6. Pay or request an invoice for the annual $100 membership dues.
